Individuals who were formerly patients of Dr. Sahar Shafey, as well as current patients of Mary DeSane, APN, who have transitioned their mental health care to Easterseals New Jersey, should contact Valerie Krzywiec at 732-380-0390 for assistance with intake paperwork, appointment scheduling, medication refills, and related inquiries.
Individuals who have not yet transitioned their care and are interested in doing so with Easterseals New Jersey may also contact Valerie Krzywiec at 732-380-0390 for assistance.
Former patients may request copies of their medical records through February 13, 2026, by calling 732-493-1919 ext. 330.
